If your driver license is lost or stolen, apply for a replacement immediately. The replacement license fee of $25.00 will be waived if you provide a copy of the police report and there are no changes to the card. A replacement may be obtained in a State or Tax Collector issuance office or using a convenience method listed in the renewal section.
Primary identification, proof of social security number and two proofs of residential address are required if you have not been issued a Florida driver license or identification card in a field office since January 1, 2010. If you fail to report a change of address you may not receive your motor vehicle registration renewal or your driver's license renewal.
